Upgradation of Contractors to Public Health Engineering Department Class I(C)
- If the applicant is a first time user, he/she needs to click the Register Tab provided in top right corner in this page and then get registered with the necessary fields such as Full Name, Mobile No., Email id (which is not mandatory), State and Password.
- Then the applicant needs to login with the credentials, by clicking the Login Tab.
- After the applicant has logged on to the system, he/she needs to click the Apply Online tab and select the service they want to apply for and click the apply button after going through the Requirements tab.
- Applicant has to fill up the form and upload the required supporting documents.
- After successful submission of the form, an acknowledgement will be generated containing the application reference no. .The application is then forwarded to higher officials for further processes. The same will be notified over SMS and Email id of the applicant.
- An applicant can view and track the status of the application by clicking the Track button and entering the applicant’s application reference no.
- In case, if there are any discrepancies or mismatch found in the online application, the applicant needs to makes the necessary corrections in the online application or has to apply through a fresh online application. The applicant will be notified about this over SMS and Email id by the concerned official.
- If all processes are correct than a date is assigned for Physical inspection. The applicant will get notified about the inspection details over SMS and Email.
- If the Inspection is successful, the applicant will be notified to pay the requisite animal tax and application fee over SMS and Email.
- After successful submission of documents, the Certiciate will be generated and the applicant can download it from the ARTPS portal. The same will be notified to the applicant over SMS and Email.
Supporting Documents :
1) Court Fee Rs. 8.25/-2) Pan Card.
3) Up-to-date GST Registration Certificate with Return (GSTR)
4) Up-to-date Labour Licence under the Govt. of Assam
5) Employees Provident Fund (EPF) Certificate with up-to-date Challan
6) Declaration with Necessary Technical Degree Certificate ( B. Tech. / BE / AMIE or Equivalent in support of the technical qualifications of key personnel
for :- Class -I(C): 1 (one) B.E. (Civil), 2 (two) Diploma (Civil) Engineer
7) List of Tools & Plants(owned or lease/ Agreement Letter / MOU is mandatory )as mentioned below: -
(i)Truck (ii) Mixture Machine (iii) Pump set (iv) Vibrator Machine (v) Generator set and small tools and plants which would be used during execution of work. Besides other equipment (vi) Bore well Drilling Machine owned or lease/ Agreement Letter / MOU is mandatory
8) Bank Solvency Certificate (From Bank concerned).
9) The annual financial turnover from registered Chartered
Accountants (CAs) in relation to engineering works, with
a value of works averaging 50.00 Lakhs over the past
three years for Class -I(C)
10) Address Proof i.e. Electricity Bill / Voter ID / Driving License/ Bank Pass Book (Address should be same of all submitted relevant documents).
11) Police Verification Report obtained from an officer not below the rank of DSP (within 1 month prior to application). (In case any adverse report is found against anybody the Registration will be automatically terminated. No claim will be entertained in this regard.) or Valid Passport.
12) Registration Certificate of Class-II Category under
PHED
13) Documents in support of work executed during past 3
years
14) Forwarding letter as recommended by the concerned authority.
All documents must be duly self-attested.
15) FOR REGISTRATION OF CONTRACTORS OTHER THAN INDIVIDUALS, SHOULD CONTAIN THE FOLLOWING DOCUMENTS IN ADDITION TO THE ABOVE:
(A) FOR PROPRIETORSHIP FIRM REGISTRATION:
An affidavit in favour of sole proprietorship.
(B) FOR PARTNERSHIP FIRM REGISTRATION
a. Certificate copy of Registration of Firm & Society of Assam.
b. Partnership deed.
c. Power of Attorney.
(C) IN CASE OF COMPANIES REGISTRATION:
a. Registration Certificate of Companies
b. Memorandum of Articles Etc.
c. Power of Attorney.
Stipulated Time Limit : 30 (Thirty) working days from the date of submission of application
Fees :
- Court Fee Stamp worth Rs. 8.25 (Rupees Eight and paisa twenty Five only) purchased in Assam
- Fixed deposit (F.D.) for a term of a minimum of 3 (three) years for 10,000/- (ten thousand) only for unreserved and 50% for ST/ SC/ OBC/ MOBC & Unemployed Degree Holder Engineer (Individual) pledged in favour of Additional Chief Engineer (PHE). ….................................... Zone for Class - I(C)
- Service charge (through PFC/CSC) - Rs. 30.
- Printing charge (in case of any printing from PFC/CSC) - Rs. 10 per page.
- Scanning charge (in case documents are scanned in PFC/CSC) - Rs. 5 per page.
- Convenience fee - Rs. 10
